After prescribing Mounjaro to patients across Irvine and Orange County for the past two years, I have had this conversation hundreds of times. The side effects list on the package insert can look alarming. The reality, for most patients, is more manageable than they expect — provided we dose correctly and they know what to watch for.
The first two weeks are the adjustment window. Nausea is real but dose-dependent. Patients who start at 2.5mg and stay there for a full four weeks before moving up almost never have significant nausea. The patients who struggle are those who rush the dose escalation — or come in from other providers already at 5mg or 7.5mg without adequate ramp-up time.
Appetite suppression kicks in around week three for most people. This is the mechanism working. The challenge I see is that some patients interpret reduced appetite as a reason to eat fewer than 800 calories a day. That is counterproductive — we lose muscle mass, not fat. I give every patient a protein floor: 100 to 120 grams of protein daily regardless of how full they feel. Non-negotiable.
The fatigue question. Some patients feel tired, especially in the first month. In my practice this almost always traces back to one of two things: not drinking enough water, or not eating enough. Tirzepatide suppresses appetite so effectively that patients sometimes forget to eat. We check in at the 30-day mark specifically for this.
What I watch for clinically. Upper abdominal pain radiating to the back is a red flag for pancreatitis — stop the medication and call immediately. I also screen carefully before starting: patients with a personal or family history of medullary thyroid carcinoma or MEN2 syndrome are not candidates for tirzepatide.
