Common, expected, temporary

• Bruising and occasional bruise at injection sites. Resolves within 5 to 10 days. More common around the eyes and over vascular zones.
• Swelling and redness. Peaks at 24 to 48 hours. Most visible in the days after treatment.
• Mild tenderness around the injection site. Resolves within the first week.

Uncommon, managed clinically

• Palpable or visible nodules. Small firm deposits under the skin. Prevention is correct dilution, depth, and post-injection massage. Management is observation, intralesional dilute saline, or occasionally dilute steroid.
• Delayed inflammatory response. Weeks to months after the appointment in a small subset of patients. Responds to anti-inflammatory treatment.

Rare

• Vascular events. Intra-arterial placement is the most serious complication of any injectable. Risk is minimized by plane selection, cannula use where appropriate, and physician-level anatomic knowledge.

This is why physician-performed matters. The worst complications of any injectable are preventable with technique and managed with urgency — both of which depend on the clinician in the room.

Sculptra suits patients whose facial aging reflects volume loss and collagen loss — not patients looking for immediate fullness.

Good candidates

• Late 30s through 70s with visible facial hollowing.
• Patients who want a youthful appearance that builds gradually and lasts two years or longer.
• Patients willing to attend 2 to 3 treatment sessions four to six weeks apart.
• Patients who prefer stimulating collagen production over product-based fullness.

Skin type considerations

Sculptra is safe across all Fitzpatrick skin types. Because it is not light-based, there is no risk of post-inflammatory hyperpigmentation from the treatment itself. For patients with Fitzpatrick skin types 4 through 6, Sculptra is often a preferred choice over laser-based skin quality treatments where PIH risk is a factor.

Sculptra is not the right product for

• Patients with active skin infection at the treatment area.
• Patients with a history of keloid scarring or hypertrophic scarring at planned injection sites.
• Patients who want a same-day result. Hyaluronic acid filler is the correct product for that goal.
• Pregnancy and breastfeeding. Treatment is deferred.
